The enemies are higher levels, drop better loot and have different names. It's just like playing through the game again but more advanced.

Finally finished the game yesterday at 36. Played a bit of playthrough 2 and got up to 39, but is bored of it. Most likely will never play it again unless I'm really bored.

it was widely a FOTM game, that's why. Sadly during that time period, my game barely worked so all my friends that were playing either don't play anymore, or are ****ing level 50 and wouldn't be that fun to play with them anyways since i'm like level 30.
