STALKER is pretty realistic as in a lot of ways, but there are plenty of flaws there too

RADIATION DOES NOT WORK THAT WAY. GOODNIGHT.

Dwarf Fortress is realistic in terms of depth of gameplay and stuff. However the setting is like dorfs and elves and dragons.

Penumbra (any of them). Very realistic physics engine and It's extremely scary to play, yes I sad scary. I'm currently playing the Overture one. Can't even finish the ****er it creeps me out so much. That and I'm playing Sims 3.

More Info:
http://www.penumbragame.com/ageGate.php

just enter a b.s. age if your not 18. It will lock you out if your not.

EDIT:

The gameplay certainly is.

Open world.
Day and night.
One shot and you're pretty much done for unless you can heal.
Bleeding out mechanics.
You can't sprint if shot in leg.
You can't aim as well if shot in arm.

It takes around 3 or 4 body shots to kill or incapicitate an enemy, most of the time 2 headshots and maybe 1 every now and then.

For me, the most realistic game would probably have to be Darkest Hour, a modification for Red Orchestra

It seems like modern warfare makes people unaware that an assault klashnikov 1947 can knock a full grown man to the ground through a car door 150 feet away with one bullet. If you shoot them anywhere in the chest they will die, if they aren't wearing body armor. It will completely lacerate any limbs it hits.
Saying it takes two bullets to the head is ridiculous, one bullet and you can pretty much say goodbye. Even if it doesn't immediately kill you, you will most likely bleed out and or go into shock.
I wish some game developers would see this, instead of. "Oh let's make our soldiers magically heal whenever they're in cover for five seconds"
No you get shot your ****ed, that shit should stay with you until your ass is dead. Maybe people would realize how destructive guns are if they tried to rush in a new shooting game and got killed by one bullet like they should. They would be like "What the ****? That's gay, so unrealistic" Then go play halo three.

I conclude I think the question "what video game do you think is most realistic" Isn't easily answerable, mainly because that implies that some games are realistic, even though the majority of games are completely ridicules and that I haven't played a game that I could consider "realistic" in my entire life.

Maybe its because people play video games to escape reality for an hour or 2. Weirdos, right?

That by all means is perfectly reasonable, I just want the creators to admit it's unrealistic and I was just pointing out that I don't think any games are realistic.
