For me:
1.Xena the warrior princess- minotaur battle- I would hide while my big brother was playing
2.Castlevania SOTN-the catacombs-the music was so sicary
3.Alundra-for some reason I WAS afraid of the hand that selects options like saving and other stuff
I WAS afraid of these cause i was just a kid back then, now I like playing or watching scary stuff, I love the way you feel the thrill, excitement, the moment you cant sleep at night and the times you get dreams or nightmares about it and wake up in the middle of the night... though I sleep 3am the morning

Probably sounds stupid but, when I learned that the chapter titles to dead space spelled out "nicole is dead" I was genuinly unsettled.

Well you have to play limbo drunk...it does make a great drunken game...like L4D2 i can only play when im drunk now just because of how fun it is...i found it out and can no longer play sober

Bioshock (spoiler alert):
There is this part where you go inside a secret cellar to find a "power to the people" machine (it allows you to upgrade one weapon). It is very dark and the cellar is flooded, there is water dripping from the ceiling. As you walk down the stairs the first thing you will see is a "Splicer" (Crazed addict), sitting on a chair facing an corner of the cellar. The splicer is entirely white because the splicer is covered in some sort of concrete, making it a statue (in this level there are several statues like this). When you walk down the stairs you will see a long flooded hallway, at the end there is the "power to the people" machine. As you walk near it the lights start to flicker and you hear something laugh behind you, when you look back at the stairs everything is still the same, you can see the statue is still sitting in its chair facing the corner. Then you use the machine and turn around to make your way back to the stairs, only to see a statue right behind you, and the chair is now empty, it doesn't move for five seconds but after that it attacks you, this really scared the shit out of me.

I played LoZ majora's mask when I was a kid, so when I realized that every single mask was a bunch of dead people trapped in masks... i was purrrty scared.

I usually don't get scared easily, especially in games - but Far Cry 3 surprised me! I was just playing through the mission like I normally would and that's when it happened...watch for yourself:

Yeah, it doesn't look so scary like that - but when you're completely focused on the game and playing it for the first time, that part really is surprising.

While it's not technically a part of the game, MissingNo. from Pokemon Yellow can be a bit creepy if it plays its 12-minute glitch cry and your not ready for it. An example of it can be found here (skip to 14:00 for the cry):
